Caldicott Principles in Practice (CPiP)
Caldicott is a key element of the Information Governance agenda in Wales, providing a set of recommendations and principles to help ensure that person identifiable information (including that of patients, staff and service users) is adequately protected. Caldicott Assessments and Improvement Plan
The Caldicott (C-PIP) self assessment provides Information Governance and Caldicott Leads with a comprehensive tool to highlight areas where improvements are required, and a benchmark for evaluating progress. The self assessment scores were as follows:
March 2012 – 78%
May 2013 – 87%
January 2014 – 67% (objectively reviewed following internal audit report recommendation)
September 2014 – 84.26%

Service user surveys
Patient Experience Reports are provided quarterly to the Board and include analysis of surveys undertaken and patient stories.
Privacy impact assessments (in full or summary format
The Health Board will undertake Privacy Impact Assessments as new projects or services are development. Completed assessments will be added as and when they are developed.
